ADMISSIONS PROCESS
ADMISSION REQUIREMENTS
- Form 138
Submit a photocopy before the GCA
Submit a the original copy after passing the GCA
Admission Test
- 4 pcs 1x1 ID Picture, white background
- Form 137 (only if requested)
- Certificate of Good Moral Character
- Completed Application Form
- Birth Certificate (Photocopy)

- A Child should pass both the Admission Test and Interview set by the OCG Admissions.
For Pre-School
- Photocopy of Birth Certificate
Students from Foreign Schools
- Certificate of Eligibility from DepEd
- Permit to Study
- Certificate of Grade Placement from DepEd
Alien Students
- Original Copy and Photocopy of the Alien
- Certificate of Registration
- Certificate of Grade Placement from DepEd

ENROLLMENT PROCEDURES
- FOR OLD STUDENTS
- Present Report Card (Form 138), Permit to Enroll
and I.D. Card to Enrollment Adviser
- Secure a Registration Form

- Fill out the Form and submit to the Enrollment
Adviser once completed

- Approach the Assessing Officer for assessment.
​
- Pay all fees at the Cashier's window
​
- Proceed to the registrar's window for final evaluation.
​
- Go back to the Enrollment Adviser for final evaluation
- FOR NEW STUDENTS
- Present admission permit to Enrollment Adviser
- Follow enrollment procedure Nos. 2-7 for old
students

- Secure FORM 137 from previous school not later
than the first quarter of the Academic Year.
ADMISSION PROCEDURE
- Pay admission test fee at the Cashier's Office

- Submit the photocopy of Form 138 and the 4 pcs.
1 x 1 picture.

- Fill out the Admission Form

- Get Testing Form
​
- Take the GCA - ADMISSION TEST
​
- If passed, take an interview..
​
- Submit all the Admission Requirements and get the
Admission Permit in the OCG
